I have 12000 dollars in credit card debt. I know all my accounts are eventually closed if I consolidate. This affects my credit score obviously. Is this a good option for me considering I know I will be paying off credit cards forever if I don't consolidate?

Debt consolidation boosts your credit score as you repay the entire amount in small affordable monthly payments. Thus you may certainly opt for this after looking for trustworthy companies who have your best interests in mind.

Consolidating thru a debt management program or with a consolidation loan.  They both have different affects on your credit.
